Frequently Asked Questions Regarding DGMS Form Fillings
1. Questions On Age Proof Documents:
1. Applicant must submit the SSC/Matriculation Certificate, SSC/Matriculation Mark sheet, or Intermediate/Higher Secondary Certificate as proof of age.
2. Provisional Certificate, Transfer Certificate, Migration Certificate, etc., will not be considered as age proof.
2. Questions On Applicants Name, Fathers Name:
1. The name of the Applicant and his Fathers name in the application must match exactly with the SSC/Matriculation Certificate.
2. In case of minor discrepancies in the name (Applicant or Father) between the application and SSC/Matriculation Certificate, the Applicant must submit an affidavit signed by a Competent Authority with an official seal or notify the discrepancy in the Government Gazette.
3. Questions On Applicants Photograph:
1. Submit or affix a recent, attested passport-sized colored photograph with the application.
2. The Applicants name should be clearly written/printed on the front of the photograph (at the top).
3. The photograph should be attested in a way that both the face and name are visible.
4. The photograph must be attested by a Gazetted Officer, Principal, Head of the Institution, or Mine Manager. Self-attested photographs are not acceptable.
5. Old, black & white, and unclear photographs are not acceptable.
4. Questions On Academic Qualification:
1. Academic Qualification Certificates (such as SSC/Matriculation Certificates, Senior Secondary School Certificate, Diploma Certificate, Degree Certificate, or equivalent) are only valid if issued by a Board/University recognized by the Government.
2. Mark List, Provisional Certificate, Transfer Certificate, Migration Certificate submitted as Academic Qualification will not be considered valid.
5. Questions On First Aid Certificates:
1. For Statutory Certificates on exemption basis: First Aid Certificate must be valid on the date of receipt of the application.
2. For Statutory Certificates on examination basis: First Aid Certificate must be valid till the date of examination.
3. First Aid Certificates issued by State Centres or Provisional Certificates are not accepted.
4. If an Applicant submits both the old Original First Aid Certificate and a new Provisional First Aid Certificate before the old one expires, the Provisional First Aid Certificate will be accepted if it is not more than 6 months old.
5. The First Aid Certificate must be signed by the Examiner; facsimile signatures are not valid.
6. Certificates without the applicants signature or the seal of the Examiner are invalid.
6. Questions On Practical Experience Certificates:
1. Practical Experience Certificates must be in the format prescribed by DGMS and available on its website; any deviation or modification from this format makes the certificate invalid.
2. Certificates with additional information in fields where no space is provided will be considered modified and therefore invalid.
3. All fields in the prescribed format must be filled; incomplete certificates are invalid.
4. Certificates with mismatched experience periods, overwriting, corrections, tampering, or unsigned by the issuing authority without a date are invalid.
Issuing Authorities:
1. Trainees in a Mine should obtain the Practical Experience Certificate from the Mine Manager.
2. Mine Managers should obtain their certificates from the Owner or Agent of the Mine.
3. Certificates issued by any other person or without the Statutory Designation and Seal of the issuing authority are invalid.
Mine Output Requirements:
4. Practical Experience in Coal mines with an average monthly output of less than 1000 tons/month or 10000 m³/month is invalid.
5. Practical Experience in Metalliferous mines with an average daily employment of less than 60 persons (for underground mines) or 160 persons (for opencast mines) is invalid. Exceptions apply to specific cases involving the use of Heavy Earth Moving Machinery.
7. Questions On Character Certificates:
1. A valid Character Certificate must include the Name, Signature, and Seal of the issuing authority along with the date of issue.
2. Character Certificates are valid if issued within one year from the date of application (for Statutory Certificates on Exemption Basis) or the date of examination (for Statutory Certificates on Examination Basis).
3. Certificates issued by a Gazetted Officer, Principal, Head of the Institution, or Mine Manager are preferred.
8. Questions On Medical Certificates:
1. A valid Medical Certificate must include the Name, Signature, Registration Number, and Seal of the issuing Medical Practitioner along with the date of issue.
2. Certificates with facsimile signatures of the issuing Medical Practitioner are invalid.
3. Only certificates issued by a Registered Medical Practitioner (with at least an MBBS qualification) are valid.
9. Questions On Fee Payment:
1. Application fees for Statutory Certificates must be paid through the Bharatkosh portal only. Payments made by other modes are not accepted.
2. Applicants must ensure that their application form reaches the DGMS Directorate within 21 days of fee payment through Bharatkosh. Applications received after this period will not be considered.
10. Questions On Forms and Formats:
1. Application forms, supporting documents, certificates, and affidavits submitted by an applicant must follow the prescribed format provided on the DGMS website. Documents not in the prescribed format will not be accepted.
2. Any additional information added to a certificate that does not have a specific field for it will render the certificate invalid.
11. Questions On Affidavits:
1. The affidavit must be duly signed by the Competent Authority, Witnesses, and the Applicant. Missing any of these signatures invalidates the affidavit.
2. Court Fees Stamp, Date of Issue, and Stamp Seal of the Competent Authority are mandatory. Missing any of these makes the affidavit invalid.
12. Questions On Applicant Details on the Statutory Certificate:
1. The Applicants Name, Fathers Name, and Date of Birth on the Statutory Certificate issued must match the details in the SSC/Matriculation Certificate.
13. Number of Attempts for Any Examination:
1. The number of attempts for any examination is counted based on the actual appearances by the applicant after the relevant statute came into force. It does not include the number of times the applicant was called for the examination but did not appear.
2. Examinations for Restricted and Unrestricted types of the same Statutory Certificate are considered separate examinations.
3. For examinations with multiple subjects, the number of attempts is counted as one attempt per Notified Examination, regardless of the number of subjects attempted.
14. Other General Questions:
1. Applications for junior examinations (e.g., Gas testing, Sirdars, Mates, Blasters, and Engine drivers Competency Examinations) should be submitted to the Zonal Secretary of the respective DGMS Zonal office based on the applicants correspondence address.
1. Applications for senior certificates (e.g., Overmans, Foremans, Surveyors, and Managers) should be submitted to the office of the Secretary (BME), DGMS (HQ), Dhanbad.
2. Incomplete Applications:
1. Incomplete applications or documents/certificates missing essential details such as the Applicants Name, Applicants Signature with date, Signature & Seal of the Competent/Issuing Authority, Date of Issue, etc., shall not be accepted.
3. Documents for Competency Certificate on Examination Basis:
1. For applications for Competency Certificates on an Examination basis, the Character Certificate, Medical Certificate, and Affidavit must be submitted in original.
2. All other documents required to be submitted with the application shall be self-attested copies.
4. Documents for Competency Certificate on Exemption Basis:
1. For applications for Competency Certificates on an Exemption basis, all the documents required to be submitted shall be in original along with one set of self-attested copies.
5. Return of Original Documents:
1. Any original document submitted with the application, except the Character Certificate, Medical Certificate, and Affidavit, shall be returned to the applicant.
6. Reapplication After Rejection:
1. An application for a Statutory Certificate, once rejected, shall not be reconsidered. To be considered again, the Applicant must submit a fresh application along with all required documents and fees.
7. Authorization/Call Letters for Examinations:
1. The print copy of the authorization/call letters for written or oral examinations, downloaded from the official website or email sent to the applicants email ID, shall be valid if supported with an original valid Government photo ID card.
15. Regarding the Validity of Application Fee:
1. Application Submission Timeline:
1. Applicants for all Competency Certificates, Duplicate Certificates, and Medical Examinations for the renewal of certificates must ensure that their application forms reach the Directorate General of Mines Safety (DGMS) within 21 days of fee payment made through BHARATKOSH.
2. Any application received after the completion of this period shall be rejected
